Appointments

Our offices are open to serve you Monday through Friday, from 8:00 a.m. to 5:00 p.m. Appointments can be made by calling our office at 320-2483. Patients are seen by appointment only. We suggest you call our office approximately eight weeks before your annual visit is due, so we can schedule a time that is most convenient for you. If you have a problem or an emergency, we will schedule an appointment with one of our physicians as soon as possible. If you are unable to keep your appointment, please notify us as soon as possible so we can reschedule your appointment for another time.

Information Requirements

If you are a new patient, we suggest you arrive approximately 15 minutes before your scheduled appointment in order to complete the necessary paperwork. At the time of your visit, our receptionist will ask you to complete a patient information form and to sign an authorization for release of medical information, as well as authorization for your insurance company to pay the practice for charges incurred. Additionally, you may be asked to complete a medical information form. Our practice is HIPAA compliant and all information is strictly confidential.

Test Results

At the time of the visit, patients will be given a telephone number to call for pap results. Normal mammogram results are communicated to both the physician and the patient by the hospital. If there is evidence of an abnormality in your pap smear or if your mammogram results are questionable, your physician will contact you by phone to discuss a recommended follow-up plan.

Prescription and Refills

Routine prescription refills will be made during regular office hours only. To request a prescription refill, please call our office and leave a message indicating your name, date of birth, the name of the medication, the prescription number and the pharmacy name and phone number.
